Important Reminder:
Please note that we have updated the order forms below to reflect the new shipping and handling increase effective October 1st, 2008, which is $15.00 for ground shipping and $30.00 for next day shipping. We have also enhanced the forms making them interactive PDFs. Now you simply type in all the information including product quantities you would like to order and the form will calculate your total including shipping and handling fees. Then print and mail them with your payment and your order will be processed as usual.
You can order online or with a mail-in order form. To reduce administration and handling costs, many companies order a three- or six-month supply. Monthly re-orders can be administratively burdensome and cannot receive automatic "Reorder Notices." We recommend ordering extra Commuter Check vouchers to cover increased participation and new employees. Commuter Check vouchers are transferable and valid for 13 months.
